What is our organizational process for improvement?

We use the PDCA model to make improvements regarding the care for our patients. PDCA = Plan, Do, Check, Act

Plan: How should it be?

Do: What do we do and how?

Check: What has been achieved?

Act: What is still to be done

How do we choose processes for improvement?

Always high-risk, high-volume, or problem-prone processes

Incident reports

Satisfaction surveys – patient, staff, physician.

Committee / staff meetings recommendation.

Monthly chart

Patient safety rounds, Infection Control surveillance.

Outcome’s data (e.g., wounds, blood use, autopsy reports, medication use, etc.)

Human Resource information (e.g., staff comments, exits interviews).
