Sentinel Event
A sentinel event is a serious, unexpected occurrence that affects patients and results in an unanticipated death, serious physical or psychological injury, including major permanent loss of limb of function, or risk thereof, not related to the natural course of the patient’s Illness or underlying condition.
If an accident occurs that may become a Sentinel Event, immediately notify:

Risk Management reports pertinent information to assist Regulatory Compliance in their goal to ensure quality patient care.
Complete an Incident Report stating the facts of the occurrence. The hospital will promptly begin a Root Cause Analysis (RCA) following our Sentinel Event Policy. The RCA will help pinpoint what specifically went wrong and how we can fix it. Staff involved in the incident will be asked to participate in the root cause analysis. Risk Management will follow-up with staff involved in the sentinel event. The Sentinel Event policy is located in each desktop under Policymedical.net.
